Simply fill out the online form (which you can find here for the Health & Wellbeing Awards and here for the Wellness From Within Awards). Payment will be taken at the end of the form, and products must be sent to us on the day you enter the awards. Products will not be returned.

You will need to send three samples of each product entered. If you have entered the same product into multiple categories, you will need to send two for each category. Products need to be sent in as soon as you have entered. If you're sending trainers or clothing, please get in touch with us to confirm sizes. Payment will be taken at the end of the form, and products must be sent to us on the day you enter the awards. Products will not be returned.
